24hr Blood Pressure Monitoring

Cervical Screening - this is offered every 3 years to women aged 25 - 49 and every 5 years from the age of 50 - 64.

Contraception - contraceptive advice can be given by our practice nurses. If you wish to have a contraceptive coil or implant, please contact Clover Street Sexual Health Clinic on 0300 123 1678

Dementia Reviews - these are provided by our mental health nurse and dementia co-ordinator

Dispensary - available to patients who reside in Upchurch and whom live more than 1.6km from another community pharmacy.

Immunisations - all children's vaccinations are offered as per the recommended national schedule as well as shingles, pneumococcal (pneumonia) and seasonal influenza vaccines. If you are concerned that you are not up to date with your vaccinations, please speak to one of the practice nurses.

Long Term Conditions Reviews - including asthma, CKD (chronic kidney disease), COPD, coronary heart disease, dementia, diabetes, hypertension (high blood pressure) and stroke / TIA.

Mental Health Reviews - these are provided by our Mental Health Nurse

Travel Advice + Vaccinations - To help us offer the appropriate service, you will be required to complete a travel risk assessment form. This should be submitted at least 6 weeks before travel. Once this has been received, one of the practice nurses will contact you (usually by email) with advice. If the travel risk assessment form is submitted with less than 3 weeks notice, we cannot guarantee an appointment for your travel vaccinations. You can download the travel risk assessment form HERE.

Dementia services - the Dementia Cafe is available on the first and third Tuesday of the month at 1.00pm - 3.00pm located in St Paul's Church, Long Catlis Road, Parkwood. Health and social professionals are in attendance to signpost as appropriate.

Young persons well-being service - provides emotional well being, mental health advice and support for young people and their families. Please call 0300 300 1981.. For young people age 18-25 25 who may be involved in self-harming behaviour or stuggling with their well-being can access support via Kent Mind & Body. Please visit www.wearewithyou.org.uk/services/mind-and-body-in-kent/. A mental health and wellbeing online platform is now also available for young people aged 11 to 25 in Medway. The service is free and can be accessed at www.kooth.com. It’s a place to get advice, information and support 24/7 as well as being able to chat to a friendly qualified counsellor Monday to Friday between 12 noon and 10pm and Saturday and Sunday between 6pm and 10pm.​ Young peole aged 16-19 can also access digital support from togetherall.com
